Here's your chance to nominate who you think is the most deserving local sports person you know, for their entry into our 'Sports Personality of the Year' awards!

Eligibility:
Residency
Nominees should be resident in Brecknockshire or Radnorshire, or represent a club or school from either shire.
Recognised Sport
Nominees should be involved with a sport recognised by the Sports Council for Wales. Visit http://sport.wales/ for further information.
Winners Exclusion
Last year’s winners are not eligible for nomination in the same category that they were victorious in.

- Nominees must be 18+ years on 31st December 2016.
- Shown great sportsmanship, motivation and determination in their sport.
- The award will be made to the individual who, in the opinion of the panel, has made an outstanding achievement in Sport at district level or higher.
- Full time professional athletes are not eligible for this award.
- Nominees must be 18+ years on 31st December 2017.
- Shown great sportsmanship, motivation and determination in their sport.
- The award will be made to the individual who, in the opinion of the panel, has made an outstanding achievement in Disability Sport at district level or higher.
- Full time professional athletes are not eligible for this award.
- Disabled athletes may also be considered for the Sports Personality of the Year Award.
*The winner of the this award will receive the Peter Gunn Trophy, presented in memory of Peter Gunn MBE.
- Nominees must be aged 14-18 on 31st December 2017
- The award will be made to the individual who, in the opinion of the panel, has made an outstanding achievement in Sport at district level or higher.
- Please provide the following:
- Sport and/or sports played
- Level the nominee is competing at and list achievements in the last 12 months giving examples of the steps taken to improve performance.
- Demonstrate the nominee’s dedication to training using examples of where they have gone beyond the norm in ensuring they progress.
- Nominees must be 18 years or under on 31st December 2017.
- The award will be made to the individual who, in the opinion of the panel, has made an outstanding achievement in Disability Sport at district level or higher.
- Please provide the following:
- Sport and/or sports played
- Level the nominee is competing at and list achievements in the last 12 months giving examples of the steps taken to improve performance.
- Demonstrate the nominee’s dedication to training using examples of where they have gone beyond the norm in ensuring they progress.
- Nominees must be 14 years or under on 31st December 2017.
- The award will be made to the individual who, in the opinion of the panel, has made an outstanding achievement in Sport in 2016.
- Please provide the following:
- Sport and/or sports played
- Level the nominee is competing at and list achievements in the last 12 months giving examples of the steps taken to improve performance.
- Demonstrate the nominee’s dedication to training using examples of where they have gone beyond the norm in ensuring they progress.
- A team of 2 or more people who have been successful at district, county, regional or national level. (Subsections of teams are not eligible).
- Teams must play in a senior league and be members of a constituted club or body related to sport.
- A commited team who have shown motivation, determination and sportsmanship throughout the season.
- A team of 2 or more aged 18 & Under on the 31st December 2017. who have participated in a team competition. (Subsections of an overall team are not eligible)
- Have been successful at district level or higher.
- A commited team that have shown motivation, determination and sportsmanship throughout their season.
- Club must be making a substantial contribution to the local community and must be actively linked to their local schools.
- Club must be soundly governed and affiliated to their National Governing Body (NGB).
- Club must have adequately qualified coaches and recognise and reward its volunteers.
- The club does not have to be performing at a high level.
- Please provide examples of the clubs achievements/progress over the last 12 months.
- Nominee must have completed a Sports Leader course e.g. Active Young People Leader.
- Nominees must be under 21 years on 31st December 2017.
- Nominee must be regularly helping to run extra curricular sports sessions in schools or volunteering in a local sports club or voluntary organisation.
- A qualified coach who has made a significant contribution to the lives of people through sport, coaching a team or individual athletes. Please list when and where the coaching takes place.
- Must hold a National Governing Body (NGB) coaching qualification.
- Current coaching qualifications and additional personal development undertaken please list.
- Is commited to developing themselves as a coach and helping others improve. Mentoring and developing other coaches. Please give evidence.
- Full time professional coaches are not eligible.
- Please list Key achievements within the last 12 months.
- Voluntarily contributes to sport and physical activity in any capacity.
- Can be volunteering at a school, sports club, youth group, voluntary organisation, community centre, sports centre, day centre, play group, etc.
- Please list achievements/commitments over the last 12 months.
- Length of time the individual has been a volunteer? (This should be a minimum of 6 months)
- Explain what role the volunteer plays. Please identify where the volunteering takes place, what role is played and how many hours are contributed.
- Describe what your nominee does in ‘going the extra mile’ and any additional achievements.
- An individual, department or organisation that has achieved a significant impact on a single school or on school sport in South Powys in 2017.
- The nominee may have helped a school achieve an increase in sports participation; may have gone above and beyond the call of duty, resulting in positive outcomes and a sustained commitment to PE & School Sport; or may be the lynchpin of a school sports event or competition in South Powys.
- Has made an outstanding contribution to sport for a minimum of 10 years as, for example, a club administrator; coach; official, umpire or referee; district, regional or national level volunteer; tournament, competition or event organiser; groundsman, green keeper or clubhouse caretaker; fundraiser; or in any sport related role.
- Please explain the role they have played, how this has affected the wider community, and what direct achievements have been the result.
